 Healthwatch County Durham has just launched a survey on the transition of care for young people from children’s to adult services. The survey is for young people aged 13 – 25yrs with extra care needs, or a family member or carer who supports them. It’s suitable for those who are just starting to think about moving or have already been transferred into adult services. The survey will run through to the end of October.
